You Answer or Decline
You get the name and reason the caller gave, and choose while they're still holding.
Or it takes a message
Decline, or don't respond in time, and the caller is told you're unavailable — and offered the chance to leave a message if you've turned that on.

Taking messages is a setting you control. Turn it off and the caller is simply thanked and told you'll hear that they called.
